The HBPC Nurse Practitioner position is located within Community Care Service, VA St. Louis Health Care System (VASTLHCS), St. Louis, Missouri. The HBPC NP is a professional nurse with advanced clinical knowledge and competencies to function in a variety of roles. The HBPC NP is accountable for the management and evaluation of patient care for a panel of frail, complex veterans living at home. He/she holds Medical Center clinical privileges and is a licensed independent provider of medical care. The NP uses evidenced-based practice and current medical care treatment guidelines while working collaboratively as the lead for an HBPC interdisciplinary team (Nursing staff, Social Worker, Rehab, Psych, Dietician, Clinical Pharmacist, Chaplain, Program/Medical Director, and expanded team to include the patient, family, caregivers, internal and community-based services involved in providing care to patients) to develop a patient driven holistic plan of care. The incumbent is accountable to adhering to established policies, standards of care, and standards of practice. Duties include, but are not limited to the following: 1. In collaboration with, and as the leader of an interdisciplinary team, the incumbent assumes primary medical management responsibility for an assigned HBPC panel of complex, chronically ill and/or severely disabled veterans in their homes and determines need for Specialty Clinic referral and/or hospitalization. Leads/facilitates weekly IDT meetings and daily staff huddles. Follows National Patient Safety Goals related to home care and follows Medical Center/Program policies. 2. Assesses the educational needs of HBPC patients, and develops, as well as implements individualized plans of care, and evaluates patient progress/response. 3. Documents and encounters progress note timely, accurately. Effectively communicates with other staff. Enters orders correctly, timely, and as clinically indicated. 4. Collaborates with other medical and health providers throughout the Medical Center and community to exchange knowledge and share clinical expertise. 5. Participates in performance improvement activities. 6. Participates in related research activities. 7. Adheres to safety regulations and reporting processes to protect and improve patient and staff safety. 8. Provides medical administrative support such as the signing of forms, death certificates, submission of referrals and/or applications to obtain care, etc. 9. Provides clinical services using tele-video technology if available and clinically indicated. The HBPC Nurse Practitioner reports directly to the HBPC Program Director, and by extension, the H&CBC Medical Director. The incumbent does not have any direct supervisory responsibilities. Work Schedule: Monday - Friday, 7:30a.m. - 4:00p.m. Financial Disclosure Report: Not required
