Provides case management and supportive services to support stability and recovery from physical and mental health, substance use, and functional concerns contributing to or resulting from homelessness. Collects data, community outreach aimed at identifying homeless and at-risk Veterans as well as building and sustaining successful community partnerships while providing supportive services. Applicants pending the completion of educational, or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
