The Palliative Care Services Psychotherapist/Social Worker works as a core member of an interdisciplinary consulting team to support patients and families dealing with serious illness. The PCSP is responsible for providing a full range of social work services including psychosocial assessment, treatment planning, therapeutic interventions, crisis intervention, resource referral, short term counseling, grief support, advance care planning, facilitation of family meetings, and collaboration with an interdisciplinary team. Working with a large multidisciplinary group, you will be responsible for conducting team-based care for a wide range
